Here it is by popular demand.  This checkout module allows your cutomers to checkout using a purchase order number.  You can even restrict this checkout method to certain customer groups.  You cal also choose the order status to be set when someone checks out using this method.  The purchase order number is saved in the transaction id field which means you can search orders by the purchase order number in the backend of the store.  Checkout the screenshots!
